If you're only sort of shy, you could stand outside her window at night (only if she knows you pretty well) and pull the Say Anything with the radio, and when she looks outside hold up a sign saying 'Will you go to prom with me?'
If you're not shy, do something bold. Run up on stage during morning announcements and ask her in front of everyone (down fall of this is if she says no you look like a retard). If you play an instrument write her a song, then sing it for her and some how slip in something about prom.

Don't wait a couple of weeks for a prom date. At least a month, if not just that f**king semester.

Just ask a girl who doesn't act like she detests you.

Also, prom is overrated.
Girls all want to go to dress up, get up-dos, and wear more make-up. They typically end up hating prom unless they're with a f**king harem of squawking ladytypes.

I got asked over myspace to my junior prom.
It was pretty cool. We were just friends but he was kind of socially awkward and I was the only female willing to wade past the silent pauses between conversation topics to try to get to know him.
Had a nice time.

I absof**kinglutely loved my senior prom. Holy sh*t go. I got the girl I had the hots for to go with me (awkwardly asked her in a parking lot after a school play. she was the one driving so if she said no that woulda....sucked). I also made a facebook group to run for prom king and we made t-shirts and sh*t and I lost by 3 votes - it was later discovered the school SGA president rigged it because she really didn't want me to win. The entire campaign was badass. Prom was great because it was just me and my hottie Kathryn. We kissed after the final song. It was beautiful. Like out of a movie beautiful.
Go to prom. I'm in tears now.

Junior prom: Had a limo. We split it between 8 of us and all payed equally (myself included.) It was pretty awesome.
Senior prom: Decided to drive. Dealt with traffic, parking, it was a mess. I don't recommend it.
Granted, my prom was at the Roosevelt hotel in Hollywood so driving there on a normal day sucks balls.

Also, be sure to keep the song brief. Don't sail in there with anything that has a 2 minute solo.

and it'll just be a song with the lyrics changed, not anything super original, cause that's hard as hell and takes a hell of a lot of time

I went stag, wore a tux with tails and got in on a group of around 5 girls and had to trade off on them. (they weren't ugly)

Even though I didn't take the girl I liked to prom, I still had quite a bit of fun and would still be regretting it if I didn't go. Honestly, even going stag is better than staying at home.

I have not read all the posts, OP, but judging from the first response, you're not getting very good advice. Here is what you do:
Hello, (insert girl's name here) how is it going?
When there is a lull in the conversation, ask her who, if anyone yet, is she going to prom with.
If answer is
If her response is "nobody" ask her, "well has anyone asked you yet?" If she then responds with no, reply with "Well how about going with me?" (be sure to smile, unless your smile is creepy) If she then looks creeped out, laugh as if you were joking, if not, be prepared for a date. If she politely refuses, laugh and say it was worth a shot.
Vary it as you will, but that's generally how it could go.

I want to ask this junior guy to it but we're 'just friends' at this point. Plus here (Canada, could just be my town), prom isn't that big of a deal; you don't really ask people out to it unless you're dating them.
Plus the guy is going to be helping out at the dry grad party afterwards, so I don't even f**king know if he COULD come, since he'd be helping out for the second half.
